    I know what 'knocked up' means in reference to females, but what does it mean here, in line 6?

    I think it just means he was knackered. Couldn't keep up with the physical demands of the job.

    By the by, in certain parts of the UK, if you knock somebody up, it just means you knock on their door. But in other parts, it means something else entirely.

    For those of us here in the US, the best slang I can think of at the moment for knackered would be wiped out or totally wiped out. And by the by would be by the way.

    Interesting how these terms differ from one side of the Atlantic to the other. And they are probably different yet again in Canada.
