A fatigue is basically a working party, a group of men assigned to do a job that is primarily physical, such as digging, hauling, etc.

I think here the author means that the personnel of the ambulance are assigned to fatigues around their camp and the hospital, which might be cleaning the patient wards, collecting and burning rubbish, cleaning their own quarters, etc. It sounds like they don't have a lot of patients coming and going at the time on this page, so fatigues may be assigned to get some extra chores done and to keep the men busy.

This seems to be a daily routine (apart from Sundays) for the Mhow ambulance along with exercising the transport horses. Hardly worth mentioning one might think.
