Why does an American lieutenant of the M.O.R.C. replace an British officer of the R.A.M.C?

Actually, it was reasonably common for american medical officers to serve with British units. There's an interesting article about them here: http://history.amedd.army.mil/booksdocs/wwi/AmericanArmyMCOfficersBEF.pdf

It took me a minute to work out that M.O.R.C. was Medical Officers Reserve Corps and, although you might think that U.S.A. means United States of America, in this case it is just United States Army.
